Your Pre-Op Assessment
Serving Seattle, Bellevue, Everett, Olympia, and Tacoma, WA
You may have a Pre-operative Evaluation done with another eye care professional but you will be responsible for their fee. If you are doing that, you are still required to complete a Pre-operative Assessment at our office before your laser eye surgery date. We are unable to confirm your candidacy for LASIK or ASA until our tests are completed.
Please arrange alternative transportation for after your pre-operative diagnostic assessment (dilated eye examination) as your vision will be blurred and you will have difficulty reading and driving.
In consideration of others and to ensure your visit is as comfortable as possible, please do not bring children with you to the office. The duration of your stay will be approximately two hours.
Bellevue LASIK cannot be held responsible for any costs incurred for:
- Travel or accommodation
- Lost employment income
- Any additional expenses incurred if you are deemed a non-candidate
- Any re-treatments, re-scheduling, or delays
For information about things to take care of before your Lasik surgery day, please see our page on Prior to Your Lasik Surgery. The Day of Your Lasik Surgery gives more detail on your procedure and how you can maximize its effectiveness.
You can download our forms ahead of time on the Preparing For Surgery page.
If you are considering a LASIK procedure, and would like more information, please contact our Lasik surgeon to set up an initial consultation. Our office is in the Seattle, WA. area. Both Dr. Leavitt and his friendly vision correction staff can answer your Lasik questions clearly and give you a tour of our comfortable facilities.
